Plan visibility rules
A plan the doctor excluded is absent from the patient’s chooser entirely — not greyed out, and with no “n options hidden” counter that would leak the size of the hidden set. The doctor’s pick carries a “Recommended for you” badge; anything else they permitted shows as “Alternative”.
